5/1 Group Disability Income Plan
When you are disabled you will receive your monthly benefits for 5 years for a covered accident or 1 year for a covered illness.
All members and their spouses who are under age 60, actively at work full time for at least 25 hours per week and live in the United States are eligible.
This plan will pay a monthly benefit amount to a maximum of $5,000 but not to exceed 70% of your monthly salary.
Future premiums will be waived for as long as benefits are payable after you have been Totally Disabled for six months.
30 day waiting period. Your monthly benefits will begin on the 31st day of your disability. |
